TSSC Technical Request Form

24 hour notice is required for any change or set-up.

Note that the TSSC is responsible for technical setup only. Furniture and room setup is the responsibility of the requesting party. For questions about this form, please call the TSSC at (801) 581-4134 or (800) 863-3496.

Will you need an in-room equipment-usage facilitator?
Do you want this event recorded?
Will you need the in-room Computer? (Dumke Conference Room Only)
Will you be bringing a laptop to use?
Will you need Internet access?
If using internet, how will you gain access?
Will you be using the projector, whether it be with a computer or anything else?
Will you need to play a DVD or VHS tape with the in-room equipment?
Will you need to use a Document Camera?
Will you need a speakerphone?
Do you need IVC or audio conference services?
If yes, you must also complete the IVC Meeting Request Form.

For equipment needs other than those listed, be sure to mention them in the comments field below

Coordinators and presenters, please try to arrive early enough before your scheduled event start time to help ensure that the TSSC can adequately set up your equipment and provide operating instructions.

TSSC may not be able to support personal settings and configurations for external laptops and non-UEN equipment used during the event. Please ensure any external equipment is in proper operating order and that presenters are comfortable with operation of the equipment.
