Entity Numbers
Your Entity Number is a unique number assigned to your organization or institution by the SLD as a means of identifying you every time you file an application or otherwise communicate with SLD. If you have never been assigned such a number, please call the SLD Client Service Bureau at 1-888-203-8100.
UEN Consortium Form 470 – 2013
UEN has submitted a Funding Year 2013 Form 470 for non-contracted services on behalf of consortium members for whom UEN is the billed entity. This FCC Form 470, number 583640001038970, is also available for reference at the USAC – Schools and Libraries' "Apply Online".
School districts, charter schools, public and private schools, and public libraries
Should post their own individual FCC Form 470 to the USAC website for Funding Year 2013 in order to qualify the eligible services for which they will be the "billed entity" during the year (July 1, 2013 – June 30, 2014). This may also be accomplished on the same USAC webpage linked above. Entity Numbers, FCCRNs, CRNs, and SPINs
Your Entity Number is a unique number assigned to your organization or institution by the SLD as a means of identifying you every time you file an application or otherwise communicate with SLD. If you have never been assigned such a number or if you need to identify a new physical facility, please call the SLD Client Service Bureau at 1-888-203-8100. UEN attempts to obtain entity numbers for Utah entities expected to require one. Please contact UEN prior to contacting USAC to see if one has already been obtained, or search for any Billed Entity Number with
USAC’s BEN Search tool.
FCC Registration Numbers
If you do not have an FCC Registration Number already connected to your USAC issued Billed Entity Number, you may easily obtain an FCCRN via the
FCC’s COmmission REgistration System (CORES).
Consultants Registration Numbers
A consultant is defined as any non-employee of the entity applying for funding that assists in filling out the application materials for a fee. Consultants may be organizations with one or more employees or they may be individuals. Anyone or any entity that is not directly employed by an eligible entity, and submits or certifies E-rate Forms on behalf of eligible entities, should be sure to, at a minimum, obtain an E-rate Letter of Agency from the eligible entities they represent prior to submitting any Forms to USAC or conducting any procurement activity on behalf of an eligible entity. Consultants must include their USAC assigned Consultant Registration Number on E-rate forms. USAC provides a comprehensive

Service Providers & Vendors
A Service Provider Identification Number (SPIN) is a unique nine-digit number assigned to your company by USAC. Service providers may want more than one SPIN if you have business units separated by type of service, or for other reasons, but you need at least one SPIN to participate in the Schools and Libraries Program. USAC provides a comprehensive

A listing of Utah State Master Contracts (SMCs) for E-rate eligible products and services that remain valid for at least some portion of funding year 2013 may be downloaded
here. Remember, if you use a SMC for 2013 Priority One services, you may need to post a 2013 Form 470 and conduct a competitive bidding process in order to legitimately identify replacement services in funding requests for the remainder of funding year 2013.
