Form 470 data includes:
- A link to Utah Education Network’s Statewide 470 for the current funding year or filing window, for Priority One services only.
- Links for obtaining Billed Entity and other Numbers from USAC, required on all E-rate forms for any E-rate applicant and used to identify locations receiving services and to report enrollment data on the FCC Form 471 Block 4.
- Links for obtaining Consultant Registration and Service Provider Identification Numbers from USAC.
