NOTE TO TEACHER:  Preassessments should not be graded.

Have each student complete the information for himself/herself and (six) family members on THE FAMILY HEALTH TREE.  Upon completion of the health tree have the student fill out the worksheet titled PREDICT YOUR OWN HEALTH FUTURE.

Discuss implications to health of a sound diet during teen years.  Emphasize connections between choices made now and the quality of life in the later life cycle stages.  See Option #1 for a follow-up activity to this preassessment.
