E-Rate Letter of Agency

The Letter of Agency (LOA) authorizes UETN to procure E-Rate eligible services and equipment and to file E-Rate forms as required by the Universal Service Administrative Company (USAC) Schools and Libraries Division (SLD). LOA forms are provided to UETN Consortium members for digital signature and are valid for specified timeframes.

UETN must have a signed LOA on file from an authorized employee of the eligible school or library before any E-Rate procurement actions or submission of applications for E-Rate discounted funding can proceed. Eligible entities must be recognized by the Utah State Board of Education (USBE) or the Utah State Library Division (USL), such as school districts, independent schools, library systems, and independent libraries.
